Here is the template for the brochure. You will probably need to have Word 2013 or newer but earlier versions should have a similar item I would think.

Here is your link for the extra credit test I showed you earlier. All you need to do is select the chapter and answer the questions. Print it out and turn it in with your notes before you take the test.
